Mayor’s Workplace Awards Application

Organizations interested in competing for the Mayor’s Workplace Awards must be MHWC Workplace Partner Certified within the last two years. For program details visit the Mayor’s Workplace Awards page of this site.

A PDF version of the Mayor’s Workplace Awards application is available for your convenience.  The deadline to apply is July 31, 2024.

The Mayor’s Workplace Awards application can be used to apply for the Mayor’s Healthiest Workplace Awards and the Moving the Needle Awards.  To be considered for the Mayor’s Healthiest Workplace Awards, applicants must submit responses in 5 out of the 6 pillars of wellness listed within the application.  To be considered for the Moving the Needle Awards, applicants can submit in 1 or more pillars of wellness listed within the application.  Applicants must be MHWC Workplace Partner Certified to be eligible for both awards.

The online application will allow you to “Save and Return” as needed. A link to the application will be provided to you via email. Your online application will only be accessible to MHWC once it has been submitted (i.e. incomplete applications cannot be accessed by MHWC).
